Registration forms

RTO 1(a) form - application for initial registration (DOC, 967KB)
The RTO 1(a) form is used when a training organisation wishes to apply for initial registration as a registered training organisation in Queensland.

Applicants for initial registration must refer to the Application guide - RTO 1(a) form - Application for initial registration.

RTO 1(b) form - application for renewal of registration (DOC, 977KB)
The RTO 1(b) form is used when an RTO wishes to apply to renew its registration as a registered training organisation.

RTOs seeking to apply to renew registration must refer to the Application guide - RTO 1(b) form - Application for renewal registration.

RTO2 form - application to amend scope of registration (DOC, 634KB)  
The RTO2 form is used to amend the scope of registration by adding courses or qualifications in existing or new training areas. Amendment to scope applications may include qualifications at an Australian Qualifications Framework level which are the same, lower or higher than the current scope of registration. The RTO2 form is also used to remove products from an RTO's scope of registration.

RTO3 form - notification guide and form (DOC, 542KB)
The RTO3 form and guide is used by Queensland-registered training organisations delivering training into other states and territories or offshore.

RTO4 form - fit and proper person requirements (DOC, 538KB)
This form is completed by senior officers, directors, substantial shareholders of applicant RTOs and RTOs to enable the registering body to assess whether each person meets fit and proper person requirements.

RTO5 form - application for approval of alternative fee protection measures (DOC, 683KB)
This form should be completed by registered training organisations (or applicants) applying for approval of alternative measure to protect student fees as required in AQTF Condition of Registration 5.

Request voluntary cancellation of registration form (DOC, 668KB)
If you wish to voluntarily cancel your registration as a registered training organisation you must apply in writing to the Training and Employment Recognition Council (TERC). Written advice must be provided by completing this form.

Update your RTO details form (DOC, 271KB)
This form is used to advise Registration Services of changes to your organisation's details. Throughout the period of registration, if any details change in regard to your organisation's legal name, trading name, responsible person, daily contact officer or business address/contact details, you must complete the attached form and post it to Registration Services. It is a condition of registration that RTOs provide the details of such changes.

Change to legal details form (DOC, 726KB)
This form is used to advise Registration Services of changes to your organisation's legal entity or structure. Some legal changes require an organisation to reapply for registration.

Conditions of registration declaration (PDF, 97KB)
This form must be submitted along with the change of legal details form. The department will contact you shortly after receiving your forms to discuss the legal changes and advise you on relevant registration requirements.

Consent to release information to the Queensland Studies Authority form (PDF, 122KB)
This form has been developed for RTOs delivering training as part of a student's compulsory phase of senior learning in accordance with the Youth Participation in Education and Training Act 2003 and the Education (Queensland Studies Authority) Act 2002.
