​User Choice VET pre-qualified supplier agreement

The User Choice VET pre-qualified supplier agreement outlines the terms and conditions for which the supplier and the department will be legally bound.

Pre-qualified supplier applicants need to complete the following instructions carefully. Organisations who submit agreements which do not fully comply with the instructions will have their agreements returned.

1. Print two (2) copies of the User Choice VET pre-qualified supplier agreement (PDF, 364KB)​.

2. Insert the following details into the User Choice VET pre-qualified supplier agreement:

  • Page 1 (Cover page) - Insert your company's legal name in the space provided
  • Page 3, Item 2
    - Insert your organisation's legal name
    - Insert your organisation's Australian Business Number (ABN)
    - Insert your organisation's business address
    - Insert the name and/or position of the legally responsible officer for your organisation
    - Insert your organisation's facsimile number
  • Page 17 - Complete one of the sections marked "for a company" or "for an individual who is not a company" with:
    - Insert your organisation's legal name
    - Insert your organisation's Australian Business Number (ABN)
    - For a company:
    • Insert the name and signature of a company director PLUS
    • Either the name and signature of a 2nd company director or secretary (unless sole Director) PLUS
    • Insert the signature of a witness to the signatures of the company directors/secretary
    or
    For an individual
    • Insert the name and signature of the individual PLUS
    • Insert the signature of a witness to the signature of the individual
    All agreements for companies must be signed by two (2) company Directors or one Director and the company Secretary, except in circumstances where a signatory is the sole Director and Secretary of a proprietary company.

3. Sign both printed Agreements, including a witness to the signature/s, and return the two original Agreements to the department as part of your application. The User Choice VET pre-qualified supplier agreement can only be signed by a legally authorised officer of the organisation.

4. Please check that all sections of the agreement detailed above are completed.

5. Faxed, photocopied or scanned and emailed versions of the User Choice VET pre-qualified supplier agreement will not be accepted. Applicants must mail two (2) complete copies with original signatures to the department.

Pre-qualified supplier status will only be granted upon receipt and approval of a registered training organisation's (RTO's) application and subsequent execution of the User Choice VET pre-qualified supplier agreement by the department.

Successful applicants will then be notified of their authority to commence training delivery against approved qualifications and receive an executed copy of the User Choice VET pre-qualified supplier agreement for their records.

Any training which commences prior to the period specified in the department's letter of authority, cannot be claimed for, or receive, payment under the User Choice 2010 - 2016 program.

Pre-qualified supplier details for approved qualifications will be published on the Queensland Training Information Service (QTIS) system by the department.
